Required Immunizations for 7th Grade
- Tdap – “1 dose of Tdap is required if at least 5 years have passed since the last dose of tetanus-containing vaccine.”
- Meningococcal (MCV4) – “1 dose of quadrivalent meningococcal conjugate vaccine is required on or after the student’s 11th birthday. If a student received the vaccine at 10 years of age, this will satisfy the requirement.”
If your child is entering 7th grade, they MUST have these vaccines in addition to all previously required vaccinations, or risk being removed from school until proof of vaccination is presented. For more information, visit the Texas Health and Human Services website.
Where to go for low-cost immunizations?
Medications at School
ANY medications to be given at school, require a physician’s order - signed by the Provider, as well as the child’s guardian. This INCLUDES OTC (over-the-counter) medications.
Additionally, ALL medications, including OTC medications, MUST be labeled by a pharmacist.
If your child has FOOD ALLERGIES, or other severe allergies, have your child’s physician complete and sign the Food Allergy and Anaphylaxis Form. Contact the nurse for the form.
PLEASE BE ADVISED: THE DISTRICT DOES NOT SUPPLY GENERIC EPI PENS. If your child has a life-threatening food allergy, please consider providing the school an EPI pen for your child.
If your child has ASTHMA, please take the Asthma Action Plan to your child’s physician for completion and signature. Please contact the nurse for the form.
For ALL OTHER CONDITIONS or illnesses which require medication be dispensed at Pin Oak during the school day, please complete either the Medication Administration – Physician Order, or if your child will self-administer, the Medication Student Self-Administration – Physician Order form. Contact the nurse for the forms.

Ms. Lebiga was born in Toronto, Canada. She graduated with a Bachelor of Science in Biology from Texas Woman’s University and received her Master of Education in School Counseling from the University of St. Thomas. She started off her education career at Dickinson ISD and eventually made her way to Houston ISD, where she now calls home. Ms. Lebiga has a daughter who she enjoys going on adventures with and making new memories. As a daughter of Filipino immigrants, she makes sure that students of all backgrounds make sure that they appropriately find their footing in today’s society by instilling the skills necessary in working towards becoming successful and self-aware global citizens. Glad to be a Charger!
